Crisis and Support Survey

Lincolnshire Partnership NHS Foundation Trust, NHS Lincolnshire – the Primary Care Trust and Independent Mental Health Forums are looking at ways to review and improve the experience of people who are or have been in mental health crisis situations.

To this end, a stakeholder group, which is made up of service users, carers, NHS staff and charitable/third sector organisations have designed the following survey as part of a project to look at new and possibly innovative ways for people and services to help in times of mental health crisis.

Please give us your ideas and we will endeavour to produce an unbiased report for future consideration.

We also hope to hold a focus group in March or April 2012 to discuss the initial survey findings.  If you would like to be included in this event, please provide us with your contact details at the end of the survey in the space provided. Places will be limited, so involvement cannot be guaranteed.

Bipolar disorder survey

Do you have Bipolar Disorder?

Would you like to take part in a research project about advance decision-making?

If so, then we would love to hear from you!

We are a team of researchers based at the University of Nottingham. We are conducting a research project to investigate whether people with bipolar disorder use the Mental Capacity Act to plan in advance for the intense phases of their condition.

In order for us to successfully conduct our research, we need your help.

We have prepared a questionnaire which will take about half an hour to complete. All your answers will be anonymous and treated with the strictest confidence.
